How to Register
1. Contact the School Secretary for a
copy of the registration form, complete that form, and mail or deliver it to the school
administration office.
2. Assemble originals of standard
health immunization records for each child attending. The Academy will need a copy
of each student's immunization record.
3. If your child is a transfer from
another school, obtain and make copies of all school transcripts or equivalent prior
academic records. The Academy will need a copy of those documents.
4. Assemble each student's
certificates of Baptism and First Communion or Confirmation, whichever is the highest
sacrament received. The school will need a clear copy of these certificates.
If you do not have one at hand, contact the parish from which the sacrament was received
for another copy.
5. Plan on attending an orientation
meeting with the school principal, date and time to be arranged with the parent, before
the first day of school. This particularly applies to new or transferring students.
Registration Requirements
Medical Requirements: As noted above, the Academy will need a clear
copy of the each student's immunization record. Typically, the form used by the
state of Colorado public schools is sufficient. The requirement is that each student
is current with all immunizations required by Colorado public health officials. This
is a common sense health requirement.
Prior School Records & Transcripts: We need copies of official school transcripts
or equivalent academic records by the first day of school. This is particularly
important for new students who have transferred from another school.
Student Handbook
Dear Parents,
After you have read and discussed the handbook policies with your children, please print
out the last two pages, the School Internet Agreement and the Acknowledgment and
Disclaimer. Please sign the forms and have your children sign also, and return the forms
to the school for their files.
Thank you.
